Immunization records for each of your children If someone outside of your household pays you, a signed letter is required. Payments from Workers Compensation Benefits Paycheck stubs dated within 30 calendar days of your appointment (example: weekly pay, bring four paycheck stubs) SNAP, Medicaid, or TANF letter (it must be dated the same month as your WIC appointment) Proof of Income required for Texas WIC appointment Letters from: a homeless shelter or a person you live with containing their signature, address and phone number and proof of their utilities. Receipts for rent or a letter from your landlord Utility (light, water, or phone) or credit card bill Take one or more of the following with you to prove where you live in Texas. TEXAS WIC EBT card and identification for each new family members that are applying. If you are currently on WIC, bring the following identification: Hospital bracelet, discharge papers or crib card If you are not currently on WIC, bring any of the following identification for each family member applying
